Print out application, complete all parts, and
return to
your high school Guidance Office
along with a copy of your most recent
transcript
and a copy of the first two pages of your
parents’ 1040
tax form.
Our deadline is May 1, but your Guidance Office may require an
earlier deadline. Please check with your Guidance
Office to be
sure.
Do not email
application.
If you are a new applicant but not currently a high school senior,
please send the application directly to The Foundation.
Print out application,
complete all parts
including personal statement, and mail to The Foundation by May 1.
Be sure to include
parents’ tax
information.
